Highly unlikely... the guy receiving the blowjob would be slightly out of focus, but his bones would still be visible. His body would NOT be one big dark mass, and his pubic hair would not be so visible either. The only part of his body that looks like it has actually been X-rayed is the hand, which makes me think that the X-ray is actually of someone whose head had to be held for the X-ray, and some kid got hold of it and decided it needed to be photoshopped.
